located at 15481 N. Jarvis Rd, Tahlequah OK 74464
Monday – Thursday from 8:00 AM to 1:30 PM.
If an interpreter is needed, please call the office at 918-456-5131 to set up an appointment. Interpreters will be available each Monday by appointment only.

NEW STUDENT ENROLLMENT
CHECK LIST
All students MUST be enrolled by a parent or legal guardian. All students MUST
enroll using the legal name on the birth certificate. Using a name other than the
one shown on the birth certificate requires legal documentation.
Documents needed for a NEW STUDENT ENROLLMENT:
□ Completed enrollment packet
□ Two (2) Proofs of Residency
1. Print out from County Assessor’s Office (located at the Cherokee County
Courthouse)
2. Current utility bill with a current date, your name and address, or a Land Tax
form with your name and address, or Rental/Lease Agreement with your
name, the physical address and the names of your children that will be
attending Grand View; or a notarized Affidavit of Living situation with your
name, physical address, and the names of all students who will be attending
Grand View PS.
□ Birth Certificate
□ Immunization Records (all immunizations must be up to date based on the
Oklahoma State Department of Health)
□ Sooner Care Number/Card (if applicable)
□ If Native American
CDIB and Citizenship/Membership Card (copy of both front and back). If the
student does not have his/her own card please include a copy of the parent(s)
CDIB card.
□ Court document for any custody/guardianship arrangements and/or all
protective orders that may pertain to your student.
Age Requirements:
Pre-Kindergarten must be four (4) years of age on or before
September 1 of the current school year
Kindergarten must be five (5) years of age on or before September 1
of the current school year
