This message is for parents of Grand View students in 6th and 8th grade ONLY.
A consent form will be sent home December 3rd or 4th describing a survey for students in 6th and 8th grade.
The survey focuses on student behaviors and beliefs. It is offered through a partnership between agencies including the Cherokee Nation, the Oklahoma State Department of Education and the Oklahoma Department of Mental Health and Substance Abuse Services.
Students will take the Oklahoma Prevention Needs Assessment Student Survey at school on December 18th and 19th. You may opt out on behalf of your child by completing and returning the form to Grand View School.
